Kybella Swelling: What to Expect Day by Day

Swelling after Kybella is expected, and it is the treatment working. In the FDA trials, 87% of patients had swelling at the injection site [1]. Most of our patients find it is at its fullest for the first few days, settles over about a week, and leaves some firmness or numbness under the chin that fades over the following weeks. Plan on roughly a week before you want to be in photos, and call us about an uneven smile, facial weakness or trouble swallowing.
Why Kybella makes you swell
Kybella is deoxycholic acid, a cytolytic drug: injected into the fat under the chin, it breaks down the membrane of fat cells so they can no longer store fat [1]. Your body then clears the debris, and the inflammation that comes with that clean-up is what you feel as swelling, tenderness and warmth. A session is a grid of small injections, up to 50 per visit, each spaced about a centimeter apart [1], so the whole treated area reacts at once rather than one spot.
Put simply: no swelling would mean no fat being cleared. The puffiness is not a complication, it is the mechanism.
Kybella swelling day by day
Everyone's chin is a little different, and a fuller area treated with more injections swells more than a small one. This is the pattern most of our patients describe.
| When | What you will notice | What to do |
|---|---|---|
| Treatment day | Stinging and warmth during the injections, then the area starts to puff up within the first hour or two. Some bruising at the needle points is common: 72% of trial patients had bruising [1]. | Go home and take it easy. Cold compresses in short intervals help with the heat and tenderness. Skip alcohol, hard exercise and heat (saunas, hot yoga) for the rest of the day. |
| Days 1 to 3 | The fullest the swelling gets. The area under the chin can look noticeably larger than before treatment, feel tight and tender, and the skin may be red. Numbness is common: 66% of trial patients had it [1]. | Sleep with your head slightly raised. Keep workouts light. This is the stretch to keep clear of video calls and photos if you can. |
| Days 4 to 7 | The swelling softens and shrinks each day. Tenderness fades, though the area can feel firm or lumpy underneath as the fat breaks down. | Most patients are comfortable in public by the end of the first week, often sooner with a scarf or a collar. Gentle activity is fine. |
| Week 2 | Little visible swelling for most people. Firmness, small nodules and some numbness under the chin are normal at this stage: 23% of trial patients had hardening and 13% had nodules [1]. | Nothing special. Resist pressing on the area to check it. |
| Weeks 3 to 6 | The firmness and numbness ease and the profile starts to read sharper as cleared fat leaves. In the trials, numbness lasted beyond 30 days for 42% of patients and swelling beyond 30 days for 20% [1], so a lingering sensation here is still within normal. | Your follow-up falls in this window. Sessions are spaced at least a month apart [1], which gives the swelling time to resolve before we assess. |
How long does Kybella swelling last?
The visible puffiness usually lasts several days to a week. The slower parts are what you feel rather than what you see: firmness and numbness under the chin can hang around for a few weeks. The label data backs that up. Injection-site reactions that lasted more than 30 days in at least one in ten trial patients were numbness (42%), swelling (20%), pain (16%) and hardening (13%) [1].
Two things change how much you swell. The first is how much fat is being treated: a larger pocket means more injections and a bigger reaction. The second is which session you are on. Later sessions treat a smaller amount of remaining fat, and in the long-term follow-up of the Kybella trials the frequency, severity and duration of side effects fell with each later session [2].
What helps the swelling settle
- Cold, not heat. Cold compresses in the first day or two ease tenderness and warmth. Avoid saunas, hot showers on the area and sunbathing while it is swollen.
- Head up at night. An extra pillow for the first few nights keeps fluid from pooling under the chin overnight.
- Easy on the body for 24 to 48 hours. Strenuous exercise and alcohol both push more blood to the area and can prolong the swelling.
- Leave the area alone. No massaging, pressing or rubbing. The firmness underneath is fat breaking down, and it does not need help.
- Pain relief as directed. Your provider will tell you what you can take for discomfort. Ask before reaching for anything that thins the blood, which can add to bruising.
- Keep your sessions spaced. Kybella is given no more often than once a month, for up to 6 treatments [1]. At Skin Med Spa most plans finish in 2 to 4 sessions.
When to call us
Ordinary swelling gets a little better every day after the first few. These are the signs that are not ordinary, and each one is a reason to call the office rather than wait for your follow-up.
- An uneven smile or weakness in your lower face. Kybella can irritate the marginal mandibular nerve, which runs along the jaw. In the trials this affected 4% of patients, and every case resolved on its own, in a median of 44 days [1]. We still want to know about it.
- Trouble swallowing. Swelling can make swallowing feel awkward for a short time. In the trials, 2% of patients had difficulty swallowing, resolving in a median of 3 days [1]. Call the same day if it is more than mild, or if it is getting worse.
- An open sore, skin breakdown or a spreading area of discoloration. Rare, and linked to injections placed too close to the skin surface [1]. This needs to be seen promptly.
- Swelling that is getting worse instead of better after the first few days, or that comes with fever, spreading redness, drainage or increasing pain, which can signal infection [1].
- A lump that is growing rather than slowly softening.
Planning your life around the swell week
The practical advice we give every Kybella patient is to book the session for a stretch when the first several days are quiet: no weddings, headshots or big presentations. A high collar, turtleneck or scarf covers the area well, and the swelling sits under the chin rather than on the face, so it is less visible from the front than you fear in the mirror.
Kybella at Skin Med Spa in Downers Grove is placed by our board-certified nurse practitioners, Barbara Kowalczyk, FNP-BC and Michael Fressola, ACNP-BC, who map the injection grid to your anatomy at the consultation and talk you through aftercare before the first vial is opened. If what you are weighing is swelling versus downtime, truSculpt iD treats a double chin with no injections and far less swelling, and the consultation is where we decide which suits your profile.
FAQ
How long does swelling last after Kybella?
The visible puffiness usually lasts several days to a week, with the first few days the fullest. Firmness and numbness under the chin can last a few weeks: in the trials, numbness lasted more than 30 days for 42% of patients and swelling for 20% [1].
Does swelling after Kybella mean it is working?
Yes. Kybella destroys fat cell membranes, and the swelling is the inflammation that comes with your body clearing those cells [1]. The amount of swelling is not a measure of your final result, though. A fuller treated area simply swells more.
Is swelling worse after the second Kybella session?
Usually milder. Later sessions treat the smaller amount of fat that remains, with fewer injections, and in the trials' long-term follow-up the frequency, severity and duration of side effects fell with each later session [2]. Sessions are spaced at least a month apart so each reaction settles first [1].
Can I hide Kybella swelling?
Mostly. The swelling sits under the chin, so a collar, turtleneck or scarf covers it and it is less noticeable from the front than in profile. Plan the first three or four days away from photos and video calls if you can.
Can I exercise after Kybella?
Keep it light for 24 to 48 hours. Hard exercise, heat and alcohol send more blood to the area and can prolong the swelling. Walking is fine from day one.
When should I worry about Kybella swelling?
Call us about an uneven smile or facial weakness, difficulty swallowing, an open sore or skin breakdown, swelling that is getting worse instead of better after the first few days, fever, or a lump that is growing. Nerve effects and swallowing trouble resolved on their own in the trials [1], but each one should be checked.
